Trauma-Informed Therapy
We work with the patient’s nervous system to address trauma and stress at the root by slowly discharging stuck stress responses (fight/flight and freeze) from the body through the use of mindfulness and body awareness, as well as processing the patient’s past and present stressors in order to provide immediate and ongoing relief.
Real-Time Biometric and Social Needs Monitoring
We provide a blood pressure cuff and wearables to collect key biometric data for at-risk patients and notify providers of any significant changes, allowing care teams to take a more proactive approach to perinatal care.
Social Work Automation
We provide support beyond medical care by making it easy for healthcare social workers/case managers to connect patients to the best resources and rapidly enroll in benefits/programs. We’ve automated the process of conducting needs assessments, referring and enrolling patients in benefits/programs.
Health Screening & Dedicated Care Team
We provide heart disease screening, lifestyle, social determinants of health, and mental health(ACES) screening to patients. We use this to personalize care plans for patients and each patient is assigned a dedicated care team of health coaches, therapists,s and nutritionists to help with their care.
